A Guide To Fill Canada Visa Application Forms

To apply for a Canadian visa, you must ensure that you need a visa before you apply, because they will not refund your visa fee if you find out that you don’t need a visa at a later date.

Each individual applicant must submit a completed application form. Children under 18 must have this form signed by their guardians.

You can decide to use the online application forms or download the paper application forms.

Ensure that you have your passport and other relevant information before you start your application.

Families should send their individual applications in one package and send this combined package to the location where their application will be processed.

To start your application, you must first create a GCKey account to login to begin your application. GCKey account simply means your username and password. The next step is to sign in to begin your application.

1. Location

The first stage of the US online visa application is to select the location where you will be applying the visa from. You must apply from your home country or other country where you are residing there legally.

2. Start application

The next stage is to click on start application to begin the application. It will then take you to the next page, and you will be asked to answer a security question. For example: “What is the given name of your mother’s mother?”. You answer and click continue to next step.

3. Surname

Write your surname as indicated in your passport. Your surname is the family name given to you. For example: my name is Samuel Gyapong. Gyapong is my surname and Samuel is my first name.

4. First/Given Names

Write the first name and given name(s) as indicated in your passport. For example: my full name is Samuel Asare Gyapong. Samuel is my first name, Asare is my middle name and Gyapong is my surname. So my given names are “Samuel Asare”.

5. Full name in Native Alphabet

Here you fill your full name in your native alphabet if applicable. There is a small box under the question you must tick if it does not apply/technology not available.

6. Have you ever used other names (i.e., maiden, religious, professional, alias, etc.) ?

You must answer with yes or no.

7. Do you have a telecode that represents your name?

You must answer with yes or no. Telecodes are four-digit code numbers that represent characters in some non-roman alphabet names.

8. Sex

Tick the box that apply to you. (Male or Female)

9. Marital Status

Tick the box that apply to you. Single or married or separated or divorced or widow. If other you specify.

10. Date and Place of birth

Fill in your date of birth in this format: (DD-MM-YY). Choose your City, State/Province, and Country as stated in your passport. Then you click next to go to the next page.

11. Country/Region of Origin (Nationality)

You must select your country of nationality as indicated in your passport.

12. Do you hold or have you held any nationality other than the one indicated above on nationality?

Answer with yes or no. If yes, select the other country/region of origin (Nationality).

13. Do you hold a passport for the other country/region of origin as indicated above?

Answer with yes or no.

14. Are you a permanent resident of a country/region other than your country/region of origin (nationality) as indicated above?

Answer with yes or no.

15. National Identification Number

Fill in your national identification number if applicable. If not applicable, you must tick the box “does not apply”.

16. US Social Security Number

Fill in your US social security number if applicable. If not, tick the box “does not apply”

17. US Taxpayer ID Number

Fill in your US taxpayer ID number if applicable. If not, tick the box “does not apply”.

18. Purpose of trip to the US

You must select the purpose of your trip among the options below and specify.

19. Have you made specific plans?

Answer with yes or no. if yes, you must provide a complete itinerary to the US: date of arrival and departure, arrival flight and departure flight, arrival city and departure city, location you plan to visit and address where you will be staying.

20. Address where you will be staying

fill in the address of where you will be staying. For example, inviting person address, hotel address, hostel address, company address etc.

21. Person/Entity paying for your trip

Select the person paying for your trip. If your trip is paid by other person or a US company or local company, you must give full details including their names, address, telephone number, email and relationship to you.

22. Are there other persons traveling with you?

You answer with yes or no. If yes, are you traveling as part of a group or organization? If yes, you must give the name of the group.

23. Have you ever been to the US?

Answer with yes or no. if yes, provide information on your last five years visit.

24. Have you ever been issued a US visa?

Answer with yes or no. if yes, give details of your previous US visa.

25. Have you ever been refused a US visa, or been refused admission to the US, or withdrawn your application for admission at the port of entry?

Answer with yes or no.

26. Has anyone ever filed an immigrant petition on your behalf with the US citizenship and immigration service?

Answer with yes or no.

27. Home address/Phone/Email information

Give full details of your home address, phone number and email address. If your home address is different from your mailing address provide your mailing address information.

28. Passport Information

Give full details of your passport information including the passport type (regular, official, diplomatic), passport number, issuing authority, city and country where the passport was issued, issuance date and expiration date and information of your stolen passport if applicable.

29. Contact person or organization in the US

Give full details of your contact person or organization in the US including full name, address, telephone number, email address and the relationship you have with the person

30. Family Information (Relatives)

Give full details of your father and mother including their names and birth dates, and you tick whether they are in US or in your home country. If you have any immediate relative excluding you parents, in the US, you must give full details of them as well.

31.Family Information (Spouse)

Give full details of your spouse information including their names, address, email, country and telephone number.

32. Present Work/Education/Training Information

Give full details of your employer or school including the name, type of occupation, address, telephone number, start date, monthly income in local currency if applicable and description of your duties etc.

33. Previous Work/Education/Training Information

Give full details of your previous work and education.

34. Do you belong to a clan or tribe?

Answer with yes or no. And write down all the languages you speak.

35. Have you traveled to any country within the last five years?

Answer with yes or no. If yes, state country(s)

36. Have you belonged to, contributed to, or worked for any professional, social, or charitable organization?

Answer with yes or no. If yes, give organization name.

37. Do you have any specialize skills or training such as firearms, explosives, nuclear, biological or chemical experience?

Answer with yes or no. If yes, explain.

38. Have you ever served in the military?

Answer with yes or no.

39. Have you ever served in, been a member of, or been involved with a paramilitary unit, vigilante unit, rebel group, guerrilla group, or insurgent organization?

Answer with yes or no. If yes, explain.

40. Security and Background Part 1

This involves questions about your health. And you must answer with yes or no. If is yes you must explain.

41. Security and Background Part 2

This involves questions about convection/crime/unlawful activities that you have been involved in. Answer with yes and no. if yes, explain.

42. Security and Background Part 3

This involves questions about export control violation and terrorist activities. Answer with yes or no. If yes, explain.

43. Security and Background Part 4

This involves questions about deportation and illegal activities. Answer with yes or no. If yes, explain.

44. Security and Background Part 5

This involves questions about violation of laws. Answer with yes or no. If yes, explain.

45. Photo

Your next step is to upload your photo. You need to be aware that, eye glasses is no longer allowed in new visa photo since 1 november, 2016.

46. Review

Review all the information filled on the application forms to ensure everything is correct before submitting your application.

47. Sign and submit

The last stage of your application process is to sign and submit. At this stage, you should make sure that your application is signed by yourself by clicking “sign and submit application” otherwise, your application will be rejected.
